• About the

Role:

• The Therapeutic Case Manager provides oversight and case managementservices to children and families who suffer from emotional, behavioraland mental health challenges in order to facilitate safety, stability,and well-being.

Provided services consist of therapeutic interventionsthat facilitate illness self-management, skill building, identificationand the use of natural supports, and use of community resources.

Services will be provided at the agency, home and community-based whennecessary.

• Principal Responsibilities:

• Provide case management, outreach, and aftercare services.

Provide basic living and social skill building, supportive counseling,collateral contact and service coordination to clients as well asconsultation, community education and training activities in ahome-based setting.

Assess clients functioning, strengths, needs, and diagnosis.

Provides appropriate therapeutic and crisis interventions.

Develop treatment plan collaboratively with client by defining goals,specific outcomes, and time frames for treatment.

• Qualifications:

• Bachelors degree in counseling, psychology, social work, or humanservices required.

Experience working with children and families, providing therapeuticintervention services.

Must be comfortable providing home visits in a variety of locationsthroughout Rutland County.

General knowledge of community resources, standard models of servicecoordination, and/or experience working in a variety of human servicessettings.

Proficient with Microsoft Word and Outlook required.

Knowledge of EHR software a plus.

Structured and organized to support those in crisis and in need.

Must have a valid drivers license, reliable transportation with personalautomobile liability insurance coverage at or above the levels mandatedby the State of Vermont.[]{#_Hlk75512964}
