PACE- Community Outreach Coordinator 3.0 3.0 out of 5 stars Jacksonville, FL 32205 Jacksonville, FL 32205 Under the direction of the PACE – Health Plan Manager, the Community Outreach Coordinator coordinates marketing and program development activities including geriatric programs, outreach programs, community, and fundraising events. The Community Outreach Coordinator assists with implementing marketing activities, attending community events, and assisting with the intake and enrollment processes as needed. Develops and maintains business contacts and relationships across the service areas with agencies and providers affiliated with the elderly and participates in the interdisciplinary team (IDT) process.
Duties and Responsibilities:
Have a strong knowledge of prohibited PACE marketing guidelines, as defined in the Federal Regulations Responsible for explaining the PACE program, either by telephone, electronically, or in-person to interested persons including candidates for enrollment, caregivers, healthcare providers/partners or others in the community interested in the PACE program. Assist in the development of internal and external communications Assist with coordinating and implementing participant satisfaction surveys Collaborates with the Center Director and the Health Plan Manager to ensure adherence to PACE Partners marketing plan and implements the marketing plan with the development of activities and a yearly timeline of events Assists Center Director in the development and managing of marketing operational budget Implements outreach of marketing efforts within all counties served Ensuring compliance within all State and Federal regulations for marketing and intake processes Develops and maintains consistent contact with existing and potential referral resources Responsible for the inventory of all marketing supplies, stock, and equipment for promotional purposes Continued growth in census as established within the intake and marketing plan Assists with data management that supports and identifies referral resources, intake process improvements and growth Builds and maintains effective collaborative relationships with area agencies and providers affiliated with the elderly and supplemental target market Acts as liaison between PACE Place and general public through exhibits and speakers’ bureau Functions as the representative for PACE Partners o Adheres to all State, Federal, and Company regulations and policies pertaining to marketing activities for the PACE program. o Comply with all state and/or federal regulations to ensure all complaints regarding quality of care or service delivery are properly processed as a grievance. o Always provides excellent customer service to internal and external customers. Qualifications and Requirements:
Education/Training/Certifications Bachelor’s degree required in Marketing, Social Services, or other health related field Minimum three-years’ experience within marketing, public relations, or social services type activities Minimum two-years of documented sales success, preferably in selling services to medical professionals within the Jacksonville area Clinical background / No licensures required Must be medically cleared of communicable diseases and up to date with immunizations First Aid and CPR certification is important, but not mandatory Current State of Florida driver’s license with driving record acceptable to PACE Partners insurance carrier, and reliable transportation Customer Relationship Management (CRM) and sales force automation systems experience preferred Computer proficiency with Microsoft Office (Word, Excel, PowerPoint, and Outlook) Have 1 year of experience working with a frail or elderly population OR, if the individual has less than 1 year of experience but meets all other job requirements, the candidate must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring. Meet a standardized set of competencies for the specific position description established by the PACE organization before working independently. Skills and Knowledge Fluent English, secondary language a benefit Experience working with a Medicare/Medicaid waiver program or demonstration project preferred. Demonstrates experience in human service care delivery system Experience with service providers in a community-based setting, acute or long-term care facilities preferred Experience and knowledge of social service principles and practices relating to elderly population Working knowledge of physical, psychosocial, behavioral, and family needs of the elderly Knowledge of the Medicare and Medicaid programs Capability to relate this information to determine eligibility requirements for PACE Partners Working knowledge of service area community health and social service delivery systems and aging networks Effective written and oral communication skills Strong interpersonal skills Ability to effectively present program model and philosophy to individuals, groups, and families Strong communication skills with ability to listen Ability to communicate and articulate PACE Partners services and benefits to medical and senior services professionals in a clear and concise manner Excellent presentation skills for both small and large audiences Comply with all state and/or federal regulations to ensure all complaints regarding quality of care or service delivery are properly processed as a grievance.
