PRIMARY RESPONSIBLITY

Responsible for coordinating community prevention services directly with various organizations (Schools, Churches, Hospitals, Shelters, etc.) within Miami-Dade and Monroe Counties through referrals from the community at large. Deals with a diverse group of external entities and internal contacts at all levels. Independent judgment is required to plan, prioritize, and organize support and aid to help direct the course of services. Supports the organization’s mission, vision, and values while dealing with a diverse group of important external clients and internal contacts at all levels of the organization. The Community Prevention Coordinator provides support to organizations in the community to help stabilize and maintain children safety and wellbeing in the community.

Master the Florida Safety Practice Model concepts, tools and philosophy. Apply Safety Methodology knowledge and use to guide decisions and recommendations on cases and when presenting a case to provide consultations Coordinate Community Prevention programmatic activities Serve as point of contact/liaison for service providers/community partners to assist in information sharing for local community services and to resolve inquiries and issues. Provide supplemental assistance to DCF/CPIs (as needed) in locating appropriate community services/resources to assist families who are low or moderate risk. Develop positive and responsive relationship with other programs/personnel, as well as with outside agencies, (i.e. DCF, Court, etc.). Work to develop alternative diversion strategies and recommendations for children and their families. Develop TIP sheets and flow charts to assist in redirecting service providers, professionals, and para-professionals in maintaining safety and wellbeing of children in the community. Track all prevention outreach efforts and monitor all referrals on cases to ensure the service determination is appropriate. Provide monthly reports on the cases received to include the number of cases and service determination. Carry out functions delegated by the Prevention Manager and Director of Prevention Services and Intake. Participate in staffing on cases of concern that are escalated between proper parties within Citrus FCN, Community providers and/or DCF. Participate in regular meetings between DCF, Prevention Providers, Intake and other parties to resolve issues and strengthen relationships. Participate in Intake and Prevention meetings to discuss issues that impact the Prevention and Intake department. Relay complaints and problems regarding prevention in the community to Prevention Manager. Develop and maintain positive liaisons with other Center departments. Adhere to Citrus FCN Intake policies and procedure. Must be available for consultation after hours and on weekends. Performs other duties, as necessary and/or assigned.

MINIMUM REQUIREMENTS

At least two years of experience in case management or protective investigation services. Child Welfare Certification preferred.

SPECIAL SKILLS AND OTHER REQUIREMENTS

To perform the job successfully, an individual should demonstrate the following competencies to perform the essential functions of this position: Knowledge of another language (Spanish and/or Haitian-Creole) required. Mastery of the Florida Practice Model. Solution focused and detail oriented. Highly organized, ability to multitask, prioritize work load and meet deadlines. Ability to work independently or demonstrate initiative. Ability to establish and maintain effective working relationships with others, including members of outside agencies, youth and families. Work requires excellent knowledge of Social Service practices and child welfare system; plus, a highly developed Cultural Awareness and Sensitivity. Experience using tools and processes recognized in the industry (state and national). Ability to react calmly and effectively in emergency situations. Work to continue personal competency through education, outside literature, seminars, etc. Must have comprehensive knowledge of a variety of computer software applications in word processing, spreadsheets, database and presentation software, including Word, Excel, and other MS Office products to include FSFN, ASK, Mindshare, CJIS.
