Job Description

Homeless Outreach Coordinator 3.6 3.6 out of 5 stars Thompson, FL JOB The Homeless Outreach Coordinator plays a vital role in connecting unsheltered individuals and families to emergency shelter, housing resources, and essential services. This position involves direct engagement with individuals experiencing homelessness—often in non-traditional, outdoor settings—with the goal of building trusting relationships that promote stability and long-term housing solutions.

EXAMPLE OF DUTIES

Actively locate and engage with unsheltered individuals and families in a variety of settings, including encampments, streets, parks, and other public spaces.

Build rapport with clients through consistent outreach efforts and repeated contact, emphasizing trust and empathy.

Respond to community and law enforcement requests for outreach services.

Conduct comprehensive needs assessments and determine program eligibility in accordance with federal, state, and local regulations.

Facilitate entry into Lee County’s Coordinated Entry System using the Homeless Management Information System (HMIS).Assist clients in obtaining necessary documentation and services, including ID cards, birth certificates, social security benefits, and SNAP.Coordinate with local shelters, housing programs, and other community resources to secure both short-term and permanent housing.

Provide ongoing case management, including development of individualized Housing Plans and support in achieving client goals.

Collaborate closely with local stakeholders such as: Lee Health and jail re-entry programsLaw enforcement agenciesLee County SchoolsLocal nonprofits and faith-based organizationsArea businesses and community organizationsMaintain accurate records, perform data entry, and generate reports in compliance with agency protocols.

Recruit and support host sites, providing guidance on program policies and procedures.

Drive to various locations within the county to provide outreach services.

SUPPLEMENTAL INFORMATION
Interpersonal Skills:

Comfortable engaging with marginalized populations in informal, non-office-based settings.

Empathy & Patience:

Able to build compassionate and respectful relationships with individuals facing mental health, substance use, and trauma-related challenges.

Crisis Management:

Remains calm and resourceful during emergencies or high-stress situations.

Cultural Competence:

Skilled in working with individuals from diverse cultural, racial, and socioeconomic backgrounds.

Knowledge Base:

Familiarity with Housing First and harm-reduction models is strongly preferred.

Technology:

Comfortable with data entry and the use of electronic case management systems such as HMIS.Standard work hours are Monday through Friday, 8:00 a.m. to 5:00 p.m., with occasional evening or weekend shifts based on community needs.
