WHY IS THIS ROLECRITICAL?
- The Outreach Specialist (OS) plays a critical role atUpward Health.
The OS is the first point of contact with ourcompany for our patients. We count on the OS to make an excellentfirst impression and to be able to effectively communicate ourservice offering to patients. The goal of this role is to informpatients about our services and to help them get started. Ourservices are focused on improving a patient’s health, and the OShelps him or her take that first step.
- The OS manages a caseloadand utilizes a range of strategies to connect with our potentialpatients. Phone is our top strategy, and it’s important that our OSis comfortable and confident communicating by phone. There may betimes that field-based approaches are utilized as well. The OSshould be prepared to use whatever strategy is most effective. TheOS reports to our Outreach Manager.

KEYRESPONSIBILITIES:
- Conduct direct outreach to patients via phone, in-personmeetings, mailings, and other strategies Speak with patients about the role that Upward Health can playin helping them improve their health Enroll patients into Upward Health’s program and collect keydata about patients during the enrollment process Manage a personal caseload of patients and ensure that eachassigned patient is contacted using the most effectivestrategy On an as-needed basis, interact with enrolled patients toensure ongoing engagement or to deliver additional community-basedtouchpoints Participate in weekly team meetings focused on ongoingeducation and improvement Accurate and timely documentation in our company’s softwaresystem•
