The Outreach specialist position is an important part of the care team at Cynthia Day Family Center, that provides addiction treatment to pregnant and parenting women. The position is responsible for helping clients gain access to our services. As an outreach specialist you would provide state wide outreach and education on services and eligibility to community members, stakeholders and referral sources. This position also provides on the ground trainings to community partners around harm reduction, overdose prevention and other relevant topics related to the target population of pregnant and parenting women struggling with substance use services. The outreach specialist will also be launching and maintaining an alumni program to engage graduates on a regular basis and build a recovery community for clients to be a part of after discharge. Ideally the Outreach Specialist will continue to provide individual recovery monitoring and peer support to clients post discharge. This position is also responsible for data tracking and reporting related to enrolments, outcomes and continuous recovery monitoring.

Basic Qualifications:

High School Diploma, GED, or equivalent Bi- or Tri- lingual Spanish/Portuguese/English Verifiable good driving record and reliable transportation Strong oral, written and reading communication skills Ability to understand verbal and written instructions Excellent interpersonal, organizational, and problem-solving skills Respectful of people and their cultural, religious and ethnic differences Experience in providing community outreach Basic computer skills Certified Recovery Support Worker (CRSW) certification

Preferred Qualifications :

At least one year of experience providing community outreach Experience working with individuals with substance use disorders (SUD) Knowledge of Microsoft Office and Athena Health (aka Centricity) Familiarity with harm reduction and trauma informed care Experience working in a multicultural setting Lived experience highly valued Knowledge/Skills/Abilities Familiarity with the community services and resources available Resourceful and ability to track down information quickly Ability to provide culturally appropriate SUD education and information, assist people in receiving the care they need, give informal counseling and provide motivational enhancement to potential clients. Promotes community relations; anticipates and responds to the needs of others; assists others in a courteous, quick manner; treats others with care and respect maintaining privacy, confidentiality and dignity Maintain a neat, clean and professional appearance including appropriate footwear for all seasons (Dress Code Policy) Conduct oneself in a manner that promotes safety

Essential Duties and Responsibilities:

Convey the purposes and services of Cynthia Day Family Center (CDFC) to the target population and emphasize the impact that the program and service would have on the client and family members. Help individuals develop substance use disorder management plans and goals Develop positive relationships with community partners state wide, this position requires some travel to meet with community partners Identify and put into place processes that will ensure clients have easy access to treatment. Provide transportation to clients using a company vehicle Document activities and results in an effective and timely manner while strictly adhering to the policies and procedures in place Work collaboratively and effectively within a team Establish positive, supportive relationships with participants. Provide outreach to potential clients state wide, this position requires travel to high need areas to ensure screenings and education are being completed to enhance access to care. Provide SUD screenings to clients, both in person and via telehealth, and assist those who screen positive in obtaining a complete SUD assessment and access to treatment if indicated. Assist clients in overcoming barriers to obtaining needed SUD services Data tracking and reporting Develop and maintain an alumni program Provide crsw/peer support aftercare and continuous recovery monitoring to individuals that have graduated or exited the program. Facilitate communication and coordinate services between providers Motivate patients to be active, engaged participants in their treatment Effectively work with people (staff, clients, doctors, agencies, etc) from diverse backgrounds in reducing cultural and socio-economic barriers between clients and institutions Build and maintain positive working relationships with the clients, providers, nurse case managers, agency representatives, supervisors and office staff Continuously expand knowledge and understanding of community resources, substance use disorders, harm reduction and trauma informed care. Identify and apply appropriate role definition and skilled boundaries Other duties as assigned
