Job Description
Community Outreach Facilitator Rochester, City of (NH) Rochester, NH 03867 JOB Statement of DutiesThe Community Outreach Facilitator will conduct public outreach within the community, including at designated locations each week to interact with and navigate receptive Rochester residents who are primarily unhoused and/or housing insecure due to experiencing mental health difficulties, including substance use disorders toward available and appropriate provider services. This individual will collaborate with community and regional service providers for the purpose of building collaborativeworking relationships.
In addition, this individual will be responsible for outreaching high- risk unhoused and/or housing insecure residents referred to by the welfare department and/or the police department. SupervisionWorks under the general guidance and direction of the Welfare Director, establishing own work plan and priorities in accordance with standard practices with substantial responsibility for determining the sequence and timing of action; only unusual cases are referred to the supervisor. Assignments/job functions are subject to additional guidance, instruction, and supervisory review.
Incumbent has substantial independence in planning and organizing the work activities and is expected to solve through experienced judgment most problems of detail by adapting methods or interpreting instructions to resolve the particular problem. Job EnvironmentAdministrative work is performed under typical office conditions. Outreach work conditions can be atypical due to increased exposure to complex social service situations, including chronic and situational mental health difficulties. Persons can experience and exhibit forms of stress, frustration, impatience, and anger due to varied temporary or chronic mental health conditions. In addition, The Community Outreach Facilitator is normally expected to work during welfare department business hours and may be required to work outside of the standard workday. May be required to make home visits, including field-based encampment visits to clients only when necessary. Private in home and field-based outreach engagements, outside of public view shall be made in pairs in collaboration with other service providers and/or city departments.
Work requires regular contact with all city departments, county, state, federal and other resources to assist with coordinating and referrals to appropriate mental health, including substance disorder service and other resources needed to maintain or return to maximum self-sufficiency.
Has access to client confidential information that could compromise the individual, public and city reputation with legal and/or financial repercussions.
EXAMPLE OF DUTIES

Must maintain appropriate ethics and professional boundaries with current or recent clients, including refraining from performing personal financial transactions to assist clients, sharing personal contact information, and engaging in personal relationships during or outside of normal business hours, including on social networking sites or other electronic media. Whenever possible, educate and refer individuals involving mental health, including substance disorder to related resources available based on their expressed needs.
Provide limited and targeted guidance and/or case management when other resources are unavailable until a reasonable transition of case management can occur. Conducts remote outreach through virtual platforms when necessary. Public relations, including through local cable channels, social media, and other forms of mass media communications will be coordinated with Welfare Director for awareness, supervision, guidance, and/or approval. Provider and community engagement meetings should primarily focus on direct client care coordination. Any broader discussions involving other topics or stakeholders require prior communication with the Welfare Director for awareness, guidance, and potential approval before proceeding. Follow up with individuals as needed or requested by the police department and/or Welfare Director.
Will conduct targeted outreach at designated locations each week.
Will work cohesively with the welfare department, police, and outside mental health, including substance disorder providers.
Prepare monthly report with program census data and community resource and partnership development.
Attends approved trainings and workshops.
As necessitated by welfare department needs, occasional work may be performed at the main welfare department office to supplement staffing.
Other similar or related tasks as required.
Knowledge of and skills in: Excellent oral and written communications skills, including capacity to communicate complex ideas professionally and thoughtfully to a variety of audiences.
Understanding complexity and viewing situations from a broad tactical perspective. Understanding of how to support all pathways of recovery.
Conducting research, locating information related to the request and evaluating the client needs.
Strong organizational skills.
Use of typical office equipment, including personal computer equipment and software programs.
Ability to: Stay abreast of relevant City of Rochester issues and current resources related to mental health, including substance disorder needs.
Work with the welfare department, outside resources, and staff at all levels to execute welfare department and organization-wide outreach strategies.
Ability to effectively collaborate with law enforcement.
Ability to always maintain professionalism and professional boundaries.
Work independently with minimal daily supervision and be self-motivatedPrioritize workload and manage time efficiently, meet deadlines, multitask, and complete assignments in a timely manner.
Must be able to maintain strict confidentiality.
Develop, implement, and manage a relevant work plan and support all pathways of recovery. Provide statistical data regarding the numbers of individuals engaged, housing situations of those individuals, outcomes, and other attainable information at the request of the Welfare Director or City Manager.
SUPPLEMENTAL INFORMATION
Special RequirementsValid driver’s license required.
Completion of Recovery coach certification, within first year of hire required.
Physical RequirementsPosition requires the ability to operate computer keyboard and standard office equipment. Prolonged periods in a stationary position.
Physical strength to move and push object up to 30lbs. Exposure to dangerous chemicals and poisonous gases.
Must be able to move safely over uneven terrain or in confined spaces.
Occasionally ascends/descends a ladder.
Must be able to work in cold and hot environments.
Ability to communicate information and ideas so others will understand.
Ability to observe items at a close range.
Ability to adjust focus at close and distant vision.
