The Community Outreach Manager is responsible for managing and providing oversight for the Community Outreach Supervisors and Community Health Workers on the Community Outreach Team. The Manager oversees the daily telephonic and face-to-face outreach and related health promotion, health education, and retention activities for the Community Outreach staff in support of the plan’s key business objectives, including Pay4Performance, EPSDT, HEDIS, Gaps in Care, and specialized engagement with prioritized populations. In collaboration with Marketing, Member, and Community Engagement, the Community Outreach Manager also oversees CHW representation for the day-to-day activities and staffing at the Enrollee and Opportunity Wellness Center(s). The Community Outreach Manager also collaborates with Member and Community Engagement to ensure adequate support and staffing for all community and health promotion programs, events, activities, and related departmental initiatives.;
- Responsibilities
- + Provide department oversight and ensure that the supervisors regularly monitor and track individual Community Health Worker activities to ensure the quality and accuracy of the information conveyed to members (enrollees) and the documentation of those activities in the company’s systems.
+ Establish and implement appropriate policies, procedures, and practices within the department to ensure that all telephonic and face-to-face outreach is conducted consistently and complies with the MI Department of Health and Human Services contract’s rules and guidelines. + Ensure the supervisors consistently and effectively manage weekly schedules and monitor daily and weekly productivity and performance dashboards. + Complete monthly/ quarterly/annual reviews, staff development, and counseling regarding job performance with department supervisors and ensure that the supervisors do the same with their staff. + Collaborate and coordinate with the departments across the organization to build and maintain good working relationships with community leaders, advocates, and providers. + Establish and maintain relationships with network providers (hospitals, FQHCs, independent practices, etc.) to conduct wellness days, focused screening days, and block scheduling arrangements to help the plan meet its HEDIS, EPSDT, Quality measures, and gap-in-care closure goals. + Identify and implement arrangements with network providers where community outreach staff can be embedded part-time or full-time in their facilities to increase Blue Cross Complete engagement with members (enrollees) to support plan goals and priorities.; + Collaborate, coordinate, and support the Care Management team to promote health outcomes.;
- Qualifications
- + A bachelor’s degree in health care, human services, and/or a related field or equivalent work experience is required. + Minimum of 3 years of supervisory experience preferred. + Minimum of 3 years of healthcare/Medicaid experience.

- + Strong leadership skills and ability to navigate change.
+ Ability to work cross-functionally, problem-solve, and navigate efficiently through bureaucracy.; + Ability to lead and effectively manage a hybrid field and office-based call center staff. + Excellent verbal and written communication capabilities. + Working knowledge and proficiency with Microsoft Office products, including the ability to;manipulate spreadsheets and data in Microsoft Excel. + Detail-oriented with strong organizational and time management skills. + Broad knowledge about the community, specific health issues, and health and social service systems. + Scheduling, planning, and follow-up skills. + Ability to interface effectively with a diverse population. + Ability to work independently and with teams.
