- Live Healthy Cherokee, (LHC). The coordinator will align priorities with the Community Health Improvement Plan to address the local health system’s work in reducing chronic disease in the area. The Coalition Coordinator

Minimum Requirements Education Bachelor’s degree Experience 5 years’ experience in project management, data-driven strategic planning, and/or program evaluation Established experience in coalition building, development, and support Excellent written and verbal communication skills Enthusiasm and experience in cultivating relationships with various community stakeholders Ability to lead and/or facilitate meetings, some public speaking experience Strong problem-solving and conflict-resolution skills Exhibit the ability for strategic and creative thinking Proven ability to complete multiple projects with minimal direction while paying attention to detail Knowledge of local community and community resources Other Knowledge, Skills, Abilities MS Office proficiency: Word, Excel, PowerPoint, Outlook Strong organizational and time management skills Understanding of Social Determinants of Health, Community and Population Health Initiatives Preferred Requirements Preferred Education Master’s degree in Public Health, Healthcare Administration, or related field Preferred Experience Cherokee County resident
Core Job Responsibilities Coalition Leadership & Partnership Development:
Recruit and cultivate strong relationships with coalition members, partners, and stakeholders. Maintain consistent communication to ensure coalition engagement, alignment, and action. Serve as primary liaison amongst the coalition, community partners, and the HPHC initiative.
Meeting & Project Coordination:
Plan, coordinate, and facilitate coalition and work group meetings, including agenda preparation, documentation, and follow-up communication. Provide facilitation that promotes engagement and decision-making amongst stakeholders.
Strategic Planning & Alignment:
Coordinate development of the CHIP. Coordinate development and implementation the CHIP to align coalition strategies with health system population health goals and data-driven strategies to reduce chronic disease and improve community health outcomes.
Data Management & Evaluation:
Adopt and utilize a shared measurement system to assess progress toward initiative goals. Enhance data-informed decision-making by ensuring partners are aware of key metrics, targets, and indicators. Prepare regular reports and an annual summary of coalition achievements and challenges.
Resource Development & Sustainability:
Track all initiative funding, ensuring responsible allocation and reporting. Identify and pursue additional funding opportunities to support and sustain coalition activities. Develop annual advocacy agendas in collaboration with community stakeholders to influence policy and funding priorities.
Communication & Community Engagement:
Promote coalition activities, outcomes, and community health initiatives through presentations, newsletters, and public outreach. Engage diverse community voices to ensure strategies are equitable and inclusive. Other Duties as Assigned
