POSITION SUMMARY
: The Community HealthcareLiaison manages the Skilled Healthcare admissions process. Thisposition is responsible for supporting the department’s activitiesin preadmission, admission, discharge, and follow-up processes.
ESSENTIAL FUNCTIONS
: Represents the campus in a positive mannerwhile creating an environment that fosters respect and courtesy forresidents and employees alike. Works within the campus programs topromote safety and the well-being of all residents and employees.
Upholds the campus’s Mission Statement. Maintains HIPAA standardsand regards all medical or healthcare information pertaining toresidents and employees as confidential. Daily communication withthe Regional Admissions Coordinator related to transfers to thehospital, LOAs, discharges, and admissions to Community SNF.Collaborate to ensure full occupancy by looking at referrals andadmitting. Coordinate with the Regional Admissions Coordinatordaily ensuring communication with the community’s nursing team.
Conducts tours of facility with prospective residents and theirfamilies. Assists in verification of insurance for all admissions,including Medicare Part A and Medicare Advantage (if needed),Medicare Part B, and private insurance. Following companyprocedures, maintains the admissions process including admissionpacket and resident guides. All admission packets must be completedbefore a resident’s admission or time of admission. Call therepresentative prior to admission to set up a time to meet.
Benefits will be discussed prior to admission, i.e., Medicarecovers day 1-20 100% and then supplemental benefits if any, and atwhat point there will be patient responsibility. Managing andprocessing insurance verification for an ongoing stay in SNF (i.e.,notes to insurance providers for continued stay) including MedicareAdvantage updates timely. Noting the chart each time a newverification is received from Managed Care. The in the chartincludes the dates verified through and the HIPPs Code ifapplicable. Following LOA for re-admission and new authorizations,when necessary, in accordance with the Medicare A and MedicareAdvantage guidelines. Coordinating Long Term Care admissions withinthe community (i.e., IL, AL, MC, SNF). Enters Private Payadmissions into MyUnity including payer information. Preparing SNFRooms for admissions ensuring room readiness before admissionarrival (i.e. all equipment including but not limited to the bed,call bell, TV, TV remote are in good working order). Coordinatingwith other departments to ensure room readiness including generalaesthetics of the room are up to standard and other departments areprepared to service new admission. Maintain Customer Service bymeeting and greeting all new admissions to ensure they understandthe SNF operations, policies, and procedures by reviewing communityinformation including but not limited to mealtimes and key rolesand to ensure initial needs are met. Documenting initial meet andgreet in MyUnity. Conducting a Satisfaction Survey prior todischarge on day of discharge or day before discharge. Calling allresidents for a health check 72 hours post Discharge; remindingthem to call us with any needs for readmission in the next 30 days.
Outreach with local referring partners to establish positivepartnerships for admissions. Establishing partnerships withsurrounding Medicaid Facilities for Discharge Planning whenapplicable. Cross Trained to ensure admission process from allreferral sources in absence of Regional Admissions Coordinator.
Assists with Assisted Living and Memory Care tours and move-ins.
Provides administrative support to department supervisors asneeded. Remains in proper attire always and maintains a highstandard of appearance. Acknowledges and follow the Resident Billof Rights. Other duties as assigned.

The physical demandsdescribed here are representative of those that must be met by anemployee to successfully perform the essential functions of thisjob. This position is often required to read, write, stand, walk,sit, use hands to finger, handle, or feel objects, tools, orcontrols, reach with hands and arms, balance, stoop, kneel, crouch,talk and/or hear. An individual in this position will be requiredto lift or carry weight up to 25 lbs. May be exposed to minimal tomoderate noise. May be required to work extended periods of time ata computer terminal. May encounter difficult situations, includingcontact with mentally ill and deceased residents.
TRAVEL :
Localand out of state travel, including overnight stays, may be requiredfrom time to time to attend conferences, training, meetings, andprofessional development programs.
EDUCATION AND EXPERIENCEREQUIREMENTS
Associated degree in a related field desired. Officeor administrative experience required. Two years’ experience in asimilar position desired. Knowledge of MyUnity software for SkilledNursing billing or related software.
KNOWLEDGE, SKILLS, ANDABILITIES REQUIRED
Proficient computer skills in Microsoft Office(Word, Excel) plus the willingness to learn other computer programsif required. Must conduct all business in a professional manner andwith a high level of confidentiality. Ability to work with minimalsupervision.
