Clinical Liaison Position Summary:

We are seeking an experienced Clinical Liaison to assess potential patients, coordinate admissions, and build and maintain relationships with referral sources for our three skilled nursing facilities in Massachusetts. This role is key to ensuring smooth transitions for patients and supporting our census development efforts.

Clinical Liaison Responsibilities:

Assess patients for skilled nursing facility suitability and coordinate admissions with hospitals, physicians, and other providers Serve as the main contact for patients and families during the admissions process Develop and maintain relationships with hospitals, physicians, case managers, and community organizations Promote our services through marketing and outreach activities Maintain accurate records and prepare referral and admission reports

Clinical Liaison Qualifications:

3+ years of experience in a similar role, preferably in skilled nursing or post-acute care Strong understanding of skilled nursing patient needs and the admissions process Excellent communication, interpersonal, and organizational skills
