Clinical Liaison & Marketer:

Build and maintain trusted relationships with hospital case managers, physicians, discharge planners, and other referral sources Identify potential admissions and guide patients and families through the referral and transition process with care and professionalism Represent and promote our facility’s services, specialties, and programs to healthcare professionals and community partners Collaborate closely with admissions and nursing teams to ensure a smooth and timely transfer of care Track referrals, monitor admissions data, and adjust outreach strategies to enhance referral outcomes

Who We’re Looking For:

Minimum of 5 years’ experience in sub-acute, long-term care, or acute care settings Strong existing relationships with local hospitals and healthcare professionals (preferred) Excellent communication, organizational, and relationship-building skills Ability to work independently, adapt to changing priorities, and manage a flexible schedule including occasional remote weekend support
