Hospital Liaison –

Long-Term Care Facility Position Summary:

The Hospital Liaison serves as the primary point of contact between hospitals and the long-term care (LTC) facility. This role is responsible for building strong relationships with hospital discharge planners, case managers, and other healthcare professionals to ensure smooth and timely transitions of patients from hospital settings to the LTC facility. The liaison also plays a key role in census development, marketing, and patient advocacy.

Key Responsibilities:

Collaborate with hospital discharge planners, case managers, and social workers to identify patients appropriate for LTC placement. Perform pre-admission assessments to determine patient eligibility and clinical needs. Educate hospital staff and families about the LTC facility’s services, amenities, and admission processes. Coordinate patient transfers, including gathering medical records, insurance information, and ensuring transportation arrangements. Maintain ongoing communication with hospital staff and facility admissions team to facilitate seamless transitions. Represent the LTC facility professionally during on-site hospital visits and community events. Track and report referral trends, admissions data, and marketing efforts to leadership. Ensure all admissions comply with regulatory standards and facility policies.

Qualifications:

Previous experience in hospital discharge planning, case management, admissions, or LTC setting strongly preferred. Strong understanding of Medicare, Medicaid, and managed care requirements. Excellent interpersonal, communication, and organizational skills. Ability to work independently and travel between facilities/hospitals as needed. Knowledge of EMR and admission software systems.
