Hospital Liaison Hospital Liaison Responsibilities Include:

Build and maintain strong relationships with hospital discharge planners, case managers, and physicians Promote our facility’s services to hospitals and referral sources Coordinate and facilitate patient admissions from hospitals to our nursing home Ensure smooth and timely transitions of care Communicate effectively with internal departments to prepare for incoming residents

Qualifications:

Previous experience in hospital liaison, admissions, marketing, or case management preferred Knowledge of long-term care, skilled nursing, or rehabilitation services Excellent interpersonal, communication, and organizational skills Ability to travel locally to hospitals as needed Apply today, for a grateful tomorrow!
