Key Responsibilities:
Cultivate and maintain referral relationships with hospitals, physicians, and case managers Evaluate referrals for clinical and financial appropriateness Coordinate smooth transitions from hospital to SNF admission Serve as a trusted resource for families and healthcare providers Meet or exceed admission goals for assigned markets Communicate effectively with our admissions and clinical teams
Qualifications:
