Job Description
Case Manager – Jackson/Marshall North North Alabama Community Care Scottsboro, AL 35768 This salaried position comes with a sign-on bonus up to $5,000, per aligned to one year of commitment! We are seeking one full-time, salaried Case Managers to promote and protect the health and well-being of the Medicaid Recipients of the Care Program in Jackson County, AL and North Marshall County, AL. The schedule is Monday-Friday (8am-5pm). Candidate must have minimum of a Licensed Baccalaureate Social Worker (LBSW) from an accredited college. Candidate must also have a current Alabama Driver’s License and Auto Insurance (State minimum requirements).
The following are also requirements:
Two (2) years of health care experience preferred but not required Excellent Computer/Communication Skills Minimum of a Licensed Baccalaureate Social Worker (LBSW) from an accredited college o Pleasant and cooperative attitude Essential Functions Assist Medicaid Recipients with navigation of healthcare systems. Develop a team approach Care Plan including the Recipient, the Primary Care Providers and other community resources and agencies. Complete face to face visits with Recipients to build rapport and identify services or referrals needed to work towards goal attainment. Willingness to travel Job site is primarily clinic based. Travel reimbursed at Federal rate.
