Job Description:

To provide individuals, families and groups with the psychosocial support needed to cope with chronic, acute or terminal illnesses. To provide services including advising family care givers, providing patient education and counseling, and making referrals for other services. May also provide care and case management or interventions designed to promote health, prevent disease and address barriers to access to healthcare. To facilitate the discharge planning process and identify community resources to assist patients and their families. To act as a clinical resource to both social service students and the community. 1. Conducts social service assessments to determine patient needs and barriers to disease management of continued recovery. 2. Documents assessment findings in the clinical record according to established standards. 3. Implements discharge planning in collaboration with patients, significant others, interdisciplinary team and physician utilizing the appropriate discharge setting. 4. Implements appropriate social service interventions to address identified patient needs. Maintains a working knowledge of resources available. Provides and documents relevant patient/family education according to policy. 5. Participates in quality improvement initiatives and educational/research activities. 6. Acts as a clinical resource. 7. Performs other duties as assigned. Most positions require 4 weeks of orientation, minimally, up to 7 weeks, based on previous experience. Orientation is completed Monday-Friday with a dedicated preceptor. Qualifications Master’s degree in Social Work required. Work experience may NOT substitute for education requirement. Currently licensed as a Licensed Master Social Worker (LMSW) by the Alabama State Board of Social Work Examiners required. For units/departments other than UAB Hospital, certified in Basic Life Support (BLS) from the American Heart Association (AHA) is required or should be obtained within 30 days of employment. If job is in UAB Hospital, successful completion of First Five Rapid Response Basics online training required within 30 days of hire into position and must be maintained annually.
