Job Summary:

The ECD Community Health Worker plays a pivotal role in enhancing the health and well-being of children aged 0-5 and their families. This position provides vital resources, facilitates access to quality healthcare and social services, assists with insurance enrollment, and conducts impactful outreach efforts. With a focus on addressing social determinants of health, the ECD Community Health Worker will empower families to access essential resources for optimal development and health outcomes. This role involves delivering linguistically and culturally responsive health education and resources directly to communities while empowering patients to actively engage in their healthcare journey.

PRIMARY ACCOUNTABILITIES

1. Engage with patients and families, assessing needs and facilitating access to resources for social determinants of health, such as housing, food, utilities, employment, and transportation. 2. Conduct outreach and educational forums to promote age-appropriate early development and preventative education to families. 3. Facilitate access to quality health care and social services including coordinating patient appointments. 4. Provide linguistically and culturally responsive information to patients and communities. 5. Provide information, application and enrollment assistance, and assistance regarding health plan coverage or determination to individuals seeking coverage through Medicaid, All Kids/CHIP, Health Insurance Marketplace, etc. 6. Provide referrals to state and or federal agencies such as Medicaid, All Kids/CHIP, etc. as needed. 7. Complete Certified Application Counselor (CAC) training program and Medicaid Assister training program and pass exams. 8. Maintain up-to-date knowledge of available resources and programs related to early childhood development, healthcare, nutrition, housing, and other social services. 9. Assist families in accessing relevant resources based on Social Determinants of Health Screening (SDoH) ensuring closure of these referrals within a 30-day timeframe. 10. Keep track of the referrals sent and follow-up on outcomes. 11. Develop and maintain community relationships to increase awareness of health center programs. 12. Coordinate with Early Childhood Development Manager for speaking engagements, marketing and public relations activities, educational opportunities, workshops, events, and other functions to target and educate individuals of health center programs and services. 13. Participate in the development of Early Childhood Development promotional materials and social media posts under direction of Early Childhood Development Manager or Capstone Leadership. 14. Collaborate with internal and external partners to ensure coordinated care and support for families. 15. Document all efforts in Electronic Medical Record including SDoH referrals, enabling services, enrollment assistance, patient education, and outreach activities, in a timely manner. 16. Compile data and generate reports to track outreach efforts, enrollment outcomes, and impact on patient outcomes. 17. Ensure compliance with HIPAA and other privacy standards. 18. Comply with all state and federal privacy, security, and conflict of interest standards. 19. Perform other duties as assigned.

Education

  • High School diploma or equivalent.
