As a crucial member of our behavioral health team, you’ll make a daily impact by connecting people to the support systems they need to thrive.
What You’ll Be Doing:
Assess and intervene in the lives of patients facing social, emotional, financial, and environmental stressors. Conduct interviews and psychosocial assessments with patients, families, caregivers, and legal representatives to fully understand their needs and barriers. Coordinate internal and external services-including healthcare, housing, transportation, and social supports-to ensure patients receive comprehensive care. Collaborate in case reviews, tracking and evaluating progress to improve outcomes and address gaps. Partner with interdisciplinary healthcare teams to maintain alignment with treatment plans and patient goals. Facilitate discharge planning, providing tailored referrals to community and spiritual resources for ongoing support. Perform Medicaid screenings and assist patients access essential benefits.
Qualifications and requirements:
Master’s degree Required State of Michigan Licensed Master’s Social Worker (LMSW) Basic Life Support Health Care Provider (BLS-HCP) Crisis Intervention Training (within 90 days hire/transfer) 2 years related experience What we offer Fundamental to providing great care is supporting and rewarding our team.
