Work where you matter. A Brief Overview Accountable for providingmedical social casework to a wide array of patients ranging fromnewborns to geriatrics. The main focus of the Social Worker is toassist in meeting the psychosocial/continuum of care needs of thepatient and family to ensure optimum functioning. In addition todirect care services, the Social Worker shall have duties ofconsultation, education, policy and program planning, performanceimprovement tasks, advocacy and liaison with the community. Eachsocial worker has a “home” department(s) they primarily work out ofbut will also be cross-trained to other areas to assist colleaguesas needed. What you will do Interviews patient and family membersto obtain information about home environment, family systems,health history, and the patient’s personality traits. Evaluatesdata gathered in terms of the medical plan of treatment, availablesocial service programs, financial counseling, and plan a pertinenttherapy program that will provide maximum benefits and safety foreach patient. Assists patients and families to understand, accept,and follow medical recommendations with emphasis on assistingpatients to be compliant with clinical care plan. Performs avariety of services such as advising on social problems, arrangingfor transportation to clinic appointments and treatments asappropriate, arranging for durable medical equipment asappropriate, and alleviation of patient/family anxieties and fears.
Utilizes resources to assist patient to resume life in thecommunity while learning to live with a chronic illness. Preparesand updates records for each patient; prepare requireddocumentation of each case; maintain records and reports asrequired. Serves as the social work representative onmulti-disciplinary team approach to patient care. Workscollaboratively with clinical coordinators, physicians, and othermembers of the care team in providing coordinated services, whichfacilitates continuity of care for the patient. Provides educationto families regarding advance directives and assists patients andfamilies with questions regarding advance directives forhealthcare. Essential Qualifications The individual must be able towork the hours specified. To perform this job successfully, anindividual must be able to perform each essential job functionsatisfactorily including having visual acuity adequate to performposition duties and the ability to communicate effectively withothers, hear, understand and distinguish speech and other sounds.

Required Education,License/Certification, or Work Experience:
Bachelor’s in SocialWork Social Worker – Licensing Board An active license in the stateof practice
