West Hartford Public Schools seeks to fill a position for a school counselor or social worker at the REACH High School Program. This is a full-time position that serves students in grades 9-12 in a personalized environment designed to meet the unique needs of students who choose a small school setting. Valid CT certification (068 or 07) is required.
Essential Responsibilities:
Plans, delivers, and evaluates support interventions to guide students toward academic and personal success.
Instructs and guides students to effectively use key skills, including time management, executive functioning, and interpersonal skills.
Provides short-term individual and group counseling to students who are experiencing social/emotional/behavioral problems that interfere with their ability to learn.
Supports students’ academic performance and progress toward graduation by reviewing progress reports, report cards, and planning interventions as needed.
Instructs, guides, and supports students regarding college access, affordability, and admission information.
In partnership with a school-to-career coordinator, facilitates work-based learning experiences.
Serves as the school’s 504 coordinator, including reviewing and implementing 504 and intervention plans for all students.
Collaborates with the school leader and the administration to plan family events, transition-related information nights, and post-secondary planning sessions.
Coordinates and conducts school-wide activities that contribute to the social, emotional, or academic goals of the school community.
Engages in ongoing communication with families in an effort to support students as they navigate through high school.
Other duties as determined by the Administration.
