Full Job Description Position Summary/Purpose:
The purpose of this position is to provide supervision and a programmatic oversight of case management services to families, adults, youth and senior population of the Town of Branford. This position will oversee case management services, and all staff assigned and to identify individuals to target skill building techniques to achieve goals related to emotional and social independence. The Case Manager is responsible for maintaining and improving the efficiency and effectiveness of all areas under his/her control under the direction of the Director.
Essential Job Functions:

Assigns appropriate staff to all clinical and/or community referrals of individuals in need. Has overall responsibility of case management services provided. Collaborates with Social Services Coordinator in verifying requests for emergency housing or other aid by assessing applicant needs and other appropriate referrals. If appropriate, enrolls client into case management program. Works with the Social Services Coordinator, Caseworkers and other Case Managers as a team to identify, communicate and service the Branford community’s social service needs and programs. Ensures list of social services resources for individuals, families, seniors, disabled and others is current and available. Assist Branford residents who are without housing by identifying and communicating potential available resources and housing options. Directs staff in helping Branford residents who are displaced from their housing because of circumstances beyond their control. Interfaces with personnel at appropriate federal, state and local agencies for the benefit of the social service needs of the community and Branford social services programs. Provide crisis intervention services as needed and in collaboration with other Town departments. Collaborates with the Emergency Response Social Worker in assessing the mental and emotional states of individuals in incidents that do not require police assistance in the community. Develop holistic follow up services to individuals, and their local support system members (family, friends, support agents). Acts as hospital liaison for Counseling Center clients and any individual sent to hospital by the Emergency Response Social Worker and other Case Managers when needed. Provide direct case management services; keeps and maintains confidential client records, develops and maintains a list of resources available, follows up with clients via phone or in person visits; consults with other staff. Monitors federal and state regulations that are applicable to social services to ensure compliance with regulations. Assists with implementation of prevention and community programs that meet the community’s needs as part of the Counseling and Community Services Center and the Parks, Recreation and Senior Services Community Center.. Transport clients in Town vehicle to clinical and/or social services appointments, if needed. Documents all referrals and actions taken by Case Manager in the appropriate electronic record system.
- Make referrals to Branford Counseling as indicated.
Responds to requests for case management services including but not limited to, assessments of capability to execute activities for daily living.
Other Functions :
May be assigned to work with the CAN (Coordinated Access Network) System to determine if displaced individuals have applied for State aid to gain access to shelter / housing. May act as lead coordinator for the Town Juvenile Review Board including the sending out of applications to the offender and their family, the coordination of paperwork for the Review Board and assisting in the definition of restitution and the associated collection of any monies due. May be assigned to work with the Town Blight Intervention team on cases involving the Town blight ordinance. May assist potential clients with financial hardship to outline their options to pay for Agency services. Assist them in applying for State and/or Federal aid where applicable. Works with Social Service Coordinator to define and offer a payment plan that can succeed for the client and the Town..
- May assist potential clients without insurance coverage to obtain coverage. Assists them in applying for CT Husky, Medicare or private insurance via the CT Exchange as needed.

Performs varied and responsible professional and administrative duties requiring knowledge and experience in human and social services; works with adults to determine appropriate resources and services to provide.
Supervision Received:
Receives direction on case management work directly from the Director or his designee.
Minimum Required Qualifications:
The qualifications required would generally be acquired with a bachelor’s degree in social work, or related degree from an accredited college or program and over 5 years of experience with psychiatric case management services; or any equivalent combination of education, training and work experience.
Must have and maintain:
Valid CT Driver’s License.
Knowledge:
Strong knowledge of the principles and practices of social services and behavioral health services; knowledge of methods such as intake processes, advising, assessments, etc.; knowledge of State Statutes relating to behavioral health;; thorough knowledge of available town, state, federal and private resources available to all program applicants; knowledge of prevention programs; knowledge of appropriate computer systems.
Ability:
Ability to deal with distressed people; ability to recognize and identify problems; ability to prioritize problems and clients to determine which issues need to be addressed first; ability to maintain working relationships with clients, state and regional agencies, etc.; ability to enforce rules, understand regulations of programs and assist with appropriate services; ability to multitask; ability to maintain confidential records; ability to prepare reports in oral and written form; ability to use computer systems and data bases; ability to maintain a safe environment during programs.
Skill:
Good verbal and good written communication skills; aptitude for working with and explaining programs and regulations to clientele ; aptitude for working with people and maintaining effective working relationships with various groups; problem solving skills; aptitude for working with paperwork and details; skill in using the above mentioned equipment;
Job Environment:
Administrative work is performed in a moderately quiet office with regular interruptions during the day from the public via telephone or in person or at meetings. Most assessments are conducted at various locations such as recreational facilities, shopping mall, school, home, and in vehicle driving to locations under possible adverse weather conditions, including hot and cold; occasionally driving to various locations to observe or conduct programs. Requires the operation of vehicles, telephones, personal computers, copiers, facsimile machines, and other standard office equipment. Makes frequent and periodic contact with clients and service providers, Town staff, schools, police, recreation facilities, civic groups, many regional and state agencies. Communication is frequent in person, by telephone, fax, email, and in writing. Contacts require a high level of patience, understanding and confidentiality
