Responsibilities:
Hold meetings with patients/residents, families, and significant others as requested and to provide support and information so as to improve quality of life Establish relationships and maintain contact and referral flow with community based agencies/services that benefit our patient/resident population Assist the Quality Assurance Committee in developing and implementing appropriate plans of action to correct identified deficiencies, prepare assigned information for input in to the MDS computer program Qualifications & Experience Requirements for
