Job Description

Social Worker, MSW Family Promise of Brevard 114 1st St, Cocoa, FL 32922 Job Purpose Family Promise of Brevard is a 501(c)(3) nonprofit dedicated to preventing and ending homelessness for families with children. Our mission is to leverage the strengths of our community to empower and elevate families facing homelessness. We work to prevent homelessness through proactive case management and eviction prevention or move-in assistance. When families do face homelessness, we provide short-term shelter, meals, and supportive services to guide them through this challenging period. Additionally, we offer ongoing case management to ensure long-term stability and help families build a secure future in their homes. The Stabilization Coordinator is responsible for delivering, coordinating, and documenting community-based case management services to families who have recently exited homelessness, or who are currently at-risk of homelessness, ensuring services are effective, trauma-informed, culturally responsive, and focused on maintaining long-term housing stability. The Stabilization Coordinator works directly with families referred from our shelter and prevention programs and community stakeholders who directly impact each family’s overall well-being and stability. This work is performed in the community with a high degree of independence and under the supervision of the Director of Programs. Essential Job Functions Direct Services (50%)  Assess and onboard eligible referrals into the stabilization program in coordination with the Shelter and/or Prevention Coordinator. Review stabilization packet with incoming families, to include the benefits of stabilization case management and agreement of mutual expectations.  Work alongside referred families to create a housing stability plan, setting mutually agreed upon goals and identifying the steps required to reach each of those goals.  Facilitate ongoing scheduled case management sessions with families to address progress on their housing stability plans. As a practice, stabilization prioritizes and protects the independence of families.

Sessions as follows:

o Meet with families at least once per week during their first ninety days in the program and scaling based on need for 12 months assuring the family has the support and guidance of Family Promise of Brevard along the path to stability. o Implement Keys to Good Tenancy, a psychoeducational module system focused on tenancy education, through a combination of in-person and online interactions.  Perform self-sufficiency and health assessments at 3

  • 6• 9• and 12-month intervals.

 Plan and conduct exit interviews with families, to include a review of what was accomplished during the year of services and a look ahead at the next steps. Resource Connection (10%)  Connect and/or advocate for families with any necessary community resources through developed relationships with referral sources, service providers, landlords, housing development, and community members. This requires creative problem solving, informal community linkages, participating in multi-disciplinary team meetings, and when necessary, the creation of new services.  Attend community meetings on behalf of Family Promise of Brevard to collaborate with county wide services for families with children, bringing relevant information back to the programs team and agency leadership.  As needed, work with other staff to deliver resources, referrals, and workshops related to tenancy education, employment resources, financial literacy and budgeting, housing plans, and educational planning.  Leverage the organization’s resources and partnerships to holistically address the needs of families. Administrative (40%)  Maintain online case file records for every family, including documenting all case management activities and summary of services provided.  Complete accurate and timely entry and exit of families in HMIS, ensuring an exit interview occurs to garner family feedback regarding their experiences with our organization and programs.  Document self-sufficiency and health assessments at 3

 Upload information into monthly programs dashboard and provide reports/stories of impact as required for funding sources.  Participate in all staff activities: scheduled organizational meetings, trainings, and retreats.  Provide operational support to the program team as needed.  Assist with training and supervising interns and staff as needed to support the stabilization program’s work.  Speak publicly in various venues about Family Promise of Brevard and our mission. Experience & Qualifications  Minimum two years’ experience in social work or related field required.  Experience working with families and/or low-income populations preferred.  Experience in Microsoft 365 and electronic records preferred.  Ability and experience in counseling clients in a social service setting.  High level of proficiency in written and oral communications.  Aptitude to handle multiple tasks simultaneously  Capability to work effectively with a wide range of people in individual and group settings  Ability to make decisions, solve problems, delegate tasks and responsibilities as needed Education  Bachelor’s degree in social work, or related degree or experience.  Masters in social work preferred Working Conditions  Working in an office and/or community-based environment (out in the field)  Working hours for this position may require split shifts.
