Lauderdale Health and Rehab, a Skilled Nursing Facility is seeking a Full-Time Social Worker Assistant to join our team! As a Social Worker, you will assist in planning, organizing, implementing, evaluating, and directing the overall operation of the Facility’s Social Services Department in accordance with current federal, state, and local guidelines and regulations.

The Responsibilities include:

• Participate in discharge planning, development and implementation of social care plans and resident assessments.
• Interview residents and families to obtain social history.
• Work with community resources to ensure the patient has a safe plan when they transition out of the facility.
• Possess the ability to seek out new methods and principles and be willing to incorporate them into existing social services

Experience:

• Should have a minimum, one (1) year, social work experience in a health care setting working directly with individuals

MEDICAID

experience in an LTC/SNF setting preferred.
