The Fatherhood Program Case Coordinator will provide individual, group, and family services to custodial/non-custodial fathers and their families. The program mission is to support fathers to become more responsible parents through economic stability, health relationships and connections to their children. The ideal candidate has previous experience working with families and providing parenting services for men. Also, be able to collaborate with community partners to insure holistic and quality services, is committed to working in neighborhood-based services with low-income families and can work independently as a member of a team.

Major Duties and Responsibilities:

Provide strength-based assessments and set goals in partnership with fathers to create and implement individual service plans for Community Based Connections, Inc Fatherhood Program Refer, advocate, collaborate within benefits, housing, employment, education, child support, child welfare, criminal justice, mental health, substance abuse systems. Facilitate parenting group based on a provide curriculum Provide support to increase connection with children and healthy relationship with parenting and/or co-parenting. Provide crisis intervention and manage emergencies as needed. Plan coordinate and implement events and activities for participants and their families. Collaborate across Community Based Connections, Inc. programs to enhance relationships and effective practice with shared families. Attended all required supervision, team meetings, and trainings Conduct outreach and presentations in communities throughout our services area. Strong sensitivity to cultural and socioeconomic characteristics present among staff and client Committed to be an active contributor to the vision and mission of the program. Work to create and support a respectful working/learning environment. Maintains effective and efficient record keeping procedures including completing the required case file documents in a timely fashion. Communicates effectively, both orally and in writing, with families, and other program staff on a regular basis. Performs other duties and responsibilities s assigned by their supervisor.

Experience, Knowledge, Skills and Abilities:

High school diploma required, bachelor’s degree in the area of study in psychology, social work or other human services related field preferred. A minimum of 3 years’ experience in working with at-risk families or lived experience.
