Social Worker, Case Manager, MSW JAFCO Sunrise, FL 33351 Social Worker, Case Manager, MSW – JAFCO seeks a Social Worker / Case Manager with a master’s degree in social work to assist in coordinating and advocating services for children and families we serve. Paid holidays off and more! As a Social Worker with JAFCO, you will serve as the child and family’s clinician, facilitating the best possible care and services available.
Your responsibilities will include:
Providing comprehensive information and referrals for services within the South Florida developmental disabilities community. Providing counseling and case management services both in office and in client’s homes. Maintaining case documentation including bio-psychosocial assessments, case notes, treatment plans and other reports based upon Medicaid, CARF, county and state standards. Facilitating support groups as well as parent education and behavior training seminars. Our Clinical Team works full Time with some weekends and evenings required. To qualify for this Social Worker position, you must: Have a master’s degree in social work. Be proficient using Microsoft Word and Excel. Have a valid driver’s license and a clean driving record. Be able to work in the tri-county area.
