A caregiver who excels in this role will: Coordinate social work services, not limited to the oncologypopulation, to provide psychosocial assessment, counseling,resource information, and referrals to patients and families. Provide care for chronically ill patient population by applyingknowledge of stages of human development, ethnic, cultural, andspiritual influences. Demonstrate knowledge of oncology diseases and treatmentsincluding level of understanding, reactions, and expectations ofdisease process. Complete psychosocial assessments and develops short andlong-term care plans based on individual need in a timely manner . Perform ongoing assessment and continued monitoring oftreatment goals based on adjustment to chronic illness and changein functional status. Consult with other medical disciplines and staff regardingbio-psychosocial, environmental, ethnic, and cultural factors whichaffect adjustment to chronic illness and its treatment. Educate and develop clinical care planning with patients,families and multidisciplinary teams based upon mutually agreedgoals to enhance, maintain and promote optimal psychosocialfunctioning throughout chronic illness treatment and its outcomes. Partner with outpatient team, insurance, and billingspecialists in implementing policy related to social work andoutpatient issues. Assist with securing financial coverage for treatments andmedications consistent with quality patient care. Analyze and facilitate referrals to community resources andgovernmental programs including local, national health and welfareagencies including expertise in accessing assistance based onpsychosocial need. Provide crisis intervention expertise as necessary includingutilization of high-risk screening criteria for case findings andoutreach activities which may include mental health and chemicaland ETOH dependency resources. Initiate multidisciplinary team conferences regarding complexcases including advocacy for and protection of patient’s dignity,confidentiality and right and access to care. Provide effective bereavement support, therapeutic counseling ,and assistance with referrals for Palliative Disease Management,Hospice, and end of life issues. Minimum qualifications for theideal future caregiver include: Master’s degree in Social Work . Licensed Clinical Social Work (LCSW). Preferred qualificationsfor the ideal future caregiver include: Certification as Breast Patient Navigator in Breast Imaging andCancer Care (CBPN-IC) preferred if working in Oncology. Three – Five years of clinical social work and two years ofhospital Care Management experience. Knowledge of InterQual guidelines. Effective verbal, written and computer skills.
