Position Summary The Discharge Support Coordinator plays a critical role in supporting individuals recently discharged from inpatient care, helping them remain engaged in treatment and avoid unnecessary readmissions. This entry-level position bridges the gap between discharge and outpatient services by promoting continuity of care, identifying and addressing barriers, and coordinating support services for patients. Strong organizational and time management skills. Ability to work collaboratively with cross-functional teams and external stakeholders. Strong communication and documentation skills.
