Role:

Tidewell Hospice is seeking a Social Worker to support patients and families during their end-of-life journey. This role provides psychosocial support, care coordination, and resource assistance to ensure comfort and dignity for those we serve.

Key Responsibilities:

Assess patient and family needs related to coping, mental health, financial resources, and caregiver support.
Provide individual and family counseling, crisis intervention, and bereavement support.
Collaborate with the Interdisciplinary Group (IDG) to develop and manage patient care plans.
Assist with financial resources, Medicare/Medicaid, and long-term care planning.
Maintain accurate electronic medical records (EMR) and participate in IDG meetings.
Support patients and families with advance directives and memorial planning.
Educate the community on hospice care.
Participate in an on-call rotation.

Qualifications:
Education & Experience:

 Minimum one year of social work experience in a healthcare setting (internships may qualify).

Licensure:

 Must hold or be in the process of obtaining one of the following Florida licenses within 90 days of hire:
Licensed Clinical Social Worker (LCSW)
Licensed Mental Health Counselor (LMHC)
Licensed Marriage and Family Therapist (LMFT)

Other Requirements:

Valid Florida driver’s license and reliable transportation for field-based roles.
Proof of valid auto insurance.

Schedule:
