This is a Full-Time Days shift opportunity. The schedule with be M-F 8-hour shifts with no weekends required. This Social Worker will be a part of an interdisciplinary care team with the goal to enhance well being by helping meet the needs impacting a person’s life through psychosocial support and connection to appropriate resources. There is a focus on alleviating barriers and lessening the burden of medical conditions. As a Social Worker II, you will be responsible for: Assessing psychosocial needs and provides services that enhance patient functioning, emotional well-being and continued access to services and resources in the community.
