MSW Responsibilities Include:

Develops, coordinates, implements and conducts initial and on-going training to improve team member competencies in Care Coordination based on best practices within the discipline. Creating/administering yearly team member competencies. Supporting department leadership team by reporting areas of deficiency and working with the team to develop action plans to improve the quality and competency of the team members within the department. Conducting yearly review/changes of all care coordination policies. Maintaining proficiency in regulatory/compliance issues. Providing all documentation changes and participating in documentation committee. Providing any education tools required/ needed by team members and leaders. Serving as a mentor for team members. Providing orientation/ re-orientation of new/existing team members, Pay-for-Skills preceptor program, and for attending meetings as requested. Traveling within all regions using personal transportation. Performing other duties as assigned.
