Full-time, non-exempt Reports to : Connect Program Manager Summary This position is responsible for delivering Connect – Coordinated Intake and Referral (CI&R) program services that provide initial intake, outreach, and case management services to pregnant women and families with young children up to age three. CI&R serves as the central intake for all services in the community for pregnant and postpartum mothers, inter-conception care, infants, and families with young children. This position requires excellent telephone communication skills, strong computer and data entry skills, and basic knowledge of Medicaid, pregnancy services, and home visiting. Essential Duties and Responsibilities include the following: 1. Provide case management services by telephone for the clients in the Connect – Coordinated Intake and Referral (CI&R) program. Secure client consent and protect confidentiality at all times. Provide information and eligibility criteria on all programs and services available to clients/families including Healthy Start, Healthy Families, Nurse Family Partnership, CAPTA, and TEAM Dad.
Case management assessment services include:
a. Initial Intake and counseling for enrollment into a home visiting program; b. Initial counseling for enrollment in a managed care plan; c. Assisting pregnant women in finding a prenatal provider; d. Assisting families in identifying a pediatrician or family practice provider; e. Providing referrals and follow-up to home visiting programs and community resources; f. Explaining Medicaid insurance benefits, as necessary; g. Explaining WIC benefits, as necessary; h. Educating on family planning, nutrition, and mental health; i. Educating on safe sleep, car seats, Diaper Bank, community resources, prenatal and postpartum care. 2. Perform accurate, secure, and confidential data entry of client information into the Well Family data system. 3. Complete the Initial Intake and refer to appropriate home visiting programs, or community resources outside of the Healthy Start system to meet the client’s needs such as Medicaid, WIC, and Family Planning Medicaid. 4. Attend staff meetings, coalition meetings, and trainings, as required. 5. Participate in community outreach, health fairs and educational events, as needed. 6. Perform other duties as assigned by Supervisor. Education, Knowledge, and Skills Associate’s or Bachelor’s Degree in social work, psychology, human services, case management, counseling, education, public health, or a related field and one (1) year of experience in community outreach, customer service, and/or working with at-risk populations is preferred. Bi-lingual in Spanish and/or Creole preferred. Ability to prioritize work requirements and work with diverse populations. Must have a valid driver’s license, a safe driving record, reliable transportation, and carry your own car insurance. 1.
Knowledge of:
o Community, social, and healthcare resources and Medicaid o Maternal and child health variables and services o Computer software and office equipment including Microsoft Office software, internet, virtual platforms & databases, printers/copiers/fax machines o HIPAA guidelines, Coalition policies, and the Healthy Start Standards and Guidelines 2.

o Utilize multi-line telephone communication systems with excellent interpersonal skills o Communicate effectively with clients, Supervisor, and staff o Work well independently and in a team environment with supervision o Plan, organize, and coordinate work activities; demonstrate strong work ethic and reliability o Use discretion and maintain confidentiality while handling sensitive information, records, and reports o Input data into a secure data system Employment Type This is a full-time, non-exempt position.
