Help to Home supports families at risk of homelessness, providing temporary housing, life skills and resources to break the cycle of poverty.
Job Overview :
The Case Manager works with families facing housing instability, helping them develop comprehensive plans for achieving stability and self-sufficiency, all while ensuring program compliance and fostering long-term success.
Key Responsibilities:
Interview and assess clients for program eligibility, referring to other services as needed. Develop personalized short and long-term goals for each client, guiding them to achieve success. Provide case management, coaching and mentorship, ensuring clients are supported in their growth and behavior modification. Coordinate workshops on financial literacy, life management and addiction recovery. Maintain accurate case records and track client progress. Work closely with other service providers and community organizations to ensure holistic support. Prepare regular progress reports for supervisors. Enforce program policies, ensuring client compliance.
Qualifications:
Passion for helping families in need. Experience in social services or case management, with a focus on developing long-term recovery plans. Strong organizational and communication skills. Bi-lingual is a plus but not required. Knowledge of social work resources, case management principles and effective communication strategies. Ability to work independently and with a diverse clientele while handling multiple cases simultaneously.
