SOCIAL WORK ASSISTANT

Shoreside Health & Rehabilitation Center 201 NE 112th Street, Miami, FL 33161

OVERVIEW:

The social worker assistant is responsible for assisting in the organizing, conducting, and maintaining the day to day operation of the Social services Department in accordance with curent Federal, State, and local standards and established policies and procedures.

RESPONSIBILITIES

Ensures that residents are treated with dignity, kindness and respect Maintains residents privacy and confidentiality and adheres to residents bill of rights at all times Assists with planning, organizing, implementing, and evaluating the day to day social service functions in accordance with current rules, regulations and guidelines that govern the facility Interviews residents and families periodically to obtain social history and to determine social needs Assists in developing the preliminary assessments of the social needs of each resident Assists with the development of the residents social plan of care and discharge plan and ensures that social service personnel follow the plan of care Involves the residents and families in planning social services program as much as possible Ensures that the social needs of all residents are provided for with the highest quality of care Assist in arranging residents transportation as needed Applies for assistance from outside agencies as needed to provide for the needs of residents Refers residents and families to appropriate agencies when the facility does not provide the services or needs of the resident Provides information to all the residents and families regarding Medicare, Medicaid and other financial assistance programs Provides assistance to staff, community agencies, etc, as needed to solve problems or needs of the residents Maintians an excellent working environment with medical professionals, facilities and organizations Reports accidents and incidents to supervisor immediately Participates in all required meeting such as care plan, Utilization review, etc, and assists in praeparing and implementing reports, agendas and programs Participates in ongoing education and training as needed

QUALIFICATIONS

EDUCATION /

EXPERIENCE:

Prior work with elders in a community or long-term care setting is preferred The nursing home social worker must have skills in communication, assessment and social work methods and techniques. Must have a working understanding of social systems along with the ability to implement pertinent state and federal regulations. B Spanish speaker preferred.

SUPERVISORY RESPONSIBILITIES

This position has no supervisory responsibilities.

WORK ENVIRONMENT

This job operates in a health care setting. This role requires regular walking to various locations around the care center. Incumbent may be exposed to virus, disease and infection from patients and specimens in working environment. Incumbent may be required to work extended periods of time at a video display terminal. Incumbent may experience traumatic situations, including psychiatric, and deceased patients. Works beyond normal working hours and on weekends and holidays when necessary Must be able to stay or return to facility during emergency conditions

While performing the duties of this job, the employee is regularly required to talk and hear. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus This position is very active and requires standing, walking, bending, kneeling, stooping, crouching, crawling and climbing all day. The employee must frequently lift or move objects and patients weighing over 50 pounds.
