Street Outreach Worker / Case manager Hermanos de la Calle Miami-Dade County, FL Outreach Worker /
Case Manager Duties:
- Conduct outreach activities to educate and inform homeless individuals about available housing options.
- Collect data and maintain accurate records of outreach efforts and interactions with individuals.
- Provide addiction support services to homeless individuals in need, and offer treatment options.
- Assist individuals in navigating the system and accessing appropriate resources.
- Conduct motivational interviewing sessions to assess needs and develop tailored housing plans.
- Make appropriate referrals to CoC resources, provide case management services on the streets, and conductividades a warm hand-off of clients to providers.
Skills:
- Strong communication skills, both verbal and written, to effectively engage with homeless individuals on the streets from diverse backgrounds.
- Empathy and compassion for individuals facing mental health challenges or addiction issues.
- Knowledge of programs to assist the homeless population, eligibility criteria, and housing processes.
- Familiarity with addiction counseling techniques and resources.
- Ability to collect and analyze data for reporting and referral purposes.
- Excellent organizational skills to manage outreach activities and maintain accurate records.
