Social Worker Position Summary:
The Social Worker provides psychosocial support to patients and their families to help them cope with the emotional, social, and financial challenges associated with hospitalization, illness, and treatment. This role involves assessment, crisis intervention, discharge planning, and coordination of care with interdisciplinary teams.
Key Responsibilities:
+ Conduct comprehensive psychosocial assessments of patients and families. + Provide emotional support, counseling, and crisis intervention. + Collaborate with medical staff to develop and implement individualized care and discharge plans. + Connect patients and families with community resources, financial assistance, and support services. + Advocate for patient needs and rights within the healthcare system. + Document all interactions and interventions in the electronic medical record (EMR) in a timely and accurate manner. + Participate in interdisciplinary team meetings and case conferences. + Provide education to patients and families regarding illness, treatment options, and coping strategies. + Support patients dealing with end-of-life issues, including advance directives and hospice referrals.
Qualifications:
+ Master’s degree in Social Work (MSW) from an accredited institution. + Current state licensure (e.g., LCSW) as required by law. + Minimum of 1-2 years of clinical experience in a healthcare or hospital setting preferred. + Strong knowledge of community resources, healthcare systems, and discharge planning. + Excellent communication, organizational, and problem-solving skills. + Ability to work effectively in a fast-paced, multidisciplinary environment. Pay and Benefits The pay range for this position is $60000.00 – $87000.00/yr. 1.
