The Pient Aligned Care Team (PACT) Social Worker loced i1 the Primary Care Clinic areas provides medical Social Work services to the Veteran populion enrolled the Orlando VA Health Care System. Services include provision of high-quality case management, clinical trement, advocacy, and coordinion of linkages to other approprie VA or community service providers and/or agencies as needed by the Veteran and/or caregiver. This is accomplished i1 collaborion with other members of the interdisciplinary trement team, using the principles of the PACT model of care. Some of the duties include, but are not limited to; Responsible for developing a trement plan and setting achievable trement goals with the Veteran/family. Able to provide current and advanced techniques to the trement of the Veteran populion. Independently providing psychosocial support to a special Veteran populion with highly complex health and/or mental health problems and the provision of short-term, solution focused counseling with Veterans and/or their families. Demonstres a, understanding of the range of trement and skills needed for all adult pients including geriric pients reled to their physical limitions, psychological needs and age associed illnesses and conditions. Provide a full range of social work services within commonly accepted standards of social work practice which includes case work, Measurement Base Care, individual, family and group counseling, discharge planning, case management and other services as approprie. Provides educion reled to VA and community resources, entitlements, Advance Directives/Living Will and will refer Veterans/families to the approprie interdisciplinary team member for identified health educion needs. Responsible for assisting the Veteran/family in establishing the approprie referrals based on the Veteran’s preference or th of his/her surroge decision-maker. Experienced in making rapid assessments and developing crisis management plans for Veterans within the VA and community. Coordine trement and communice with other social workers and community program coordinors to include NIC, CNH, and TCM, Behavioral Health Programs, Homeless Program, Suicide Prevention etc., to facilite continuity of care for Veterans. Collabore in the performance improvement processes and comply with performance measures as required by the VA for their specific populion and/or program. Actively contribute to the success of local organizional goals within PACT and will participe in activities th obtain feedback from Veterans and/or caregivers and work to enhance services as needed. Maintains a current and productive network of referral resources, makes provision for referral services from other agencies, community resources, and other governmental agencies to assist Veterans with resolving problems or support continued transition to independence. Work Schedule :

The primary tour of duty M-F 8am to 4:30pm. However, the selectee may also be required to work various shifts on a permanent or temporary basis, based on the needs of the service and pient care.
