This role involves working with diverse populations, providing case management, and facilitating group therapy sessions. The Social Worker will play a crucial role in assessing client needs, developing treatment plans, and advocating for resources that promote well-being. Responsibilities Conduct comprehensive assessments of clients’ needs and strengths. Develop and implement individualized treatment plans tailored to each client’s circumstances. Facilitate group therapy sessions, focusing on various therapeutic techniques including motivational interviewing. Provide individual counseling to clients, addressing behavioral health issues and promoting mental wellness. Collaborate with healthcare providers, community organizations, and families to coordinate care and support services. Offer home community care services as needed, ensuring clients receive appropriate resources in their living environments. Manage case files diligently, documenting progress and interventions accurately. Engage with children and families in counseling sessions to address specific challenges and promote healthy development. Advocate for clients’ rights and access to necessary services within the community. Qualifications Experience in case management and individual/group counseling is essential. Familiarity with behavioral health practices and working with individuals on the autism spectrum is highly desirable. Strong interpersonal skills with the ability to build rapport with clients from diverse backgrounds. Excellent communication skills, both verbal and written, for effective documentation and reporting. Ability to work collaboratively within a multidisciplinary team environment.
