Job Summary:

Facilitate, monitor, document, and track the Integrated Practice Team (IPT) Process. Will facilitate the group to brainstorm ideas that can assist a family with positive change while addressing conflict and accurately route cases to the proper community resource. Utilize a family-centered and strength-based team approach with community involvement in the protection of children. Serve as a consultant to CPI’s and CMO case managers and coordinate special case staffings. Network with various community agencies and resources. Responsible for ensuring cross-system integration and interdisciplinary support. Participate with Family Preservation (FPT) Team process to review referrals by a mental health provider for wraparound intervention services. Assist team with referral processing and auditing for all programs within Family Preservation.

Essential Duties:

Lead the Integrated Practice Team (IPT) by facilitating a core group of representatives from differing organizations whose purpose is to arrange wrap around and strength based services to allow children to remain safely in their homes Maintain IPT records on the incoming correspondence, action documents, schedule, and follows up on each case to ensure timely reply or action. Maintain files and records of correspondence and enters all IPT notes and action plans into FSFN Attend Family Preservation Team (FPT) referral review meetings with IPT partnering agencies. Network with various community agencies and resources to support the IPT and Family Preservation Department Assist with processing referrals for other Family Preservation program, such as FAST, STEPS, Safety Management, and High-Risk Newborn . Perform audits of Family Preservation Programs Participate in special projects and other departmental requirements, ongoing.

Minimum Qualifications:
Education:

Bachelor’s Degree required. Certification in Facilitation/ Mediation or Master’s degree preferred.

Experience:

Three years of experience working with at-risk children and/or families. Experience in Facilitation/Mediation preferred. Licensure, Certification, and/or

Registration:

Knowledge of:

Florida Statutes pertaining to Child Welfare. Family and group dynamics with a range of intervention techniques. Best Practices as it relates to Family Preservation and Facilitation. Social Worker principles, techniques, and practices and their application to complex case work, group work, and community problems. Laws, regulations, and policies which govern all Child Welfare programs. Child Protection and child development issues. New and existing community resources and social service multi-disciplinary providers in Pasco and Pinellas counties and surrounding areas.
