Job Summary:
We are seeking a compassionate and experienced Medical Social Worker (MSW) for a per diem home health position. The ideal candidate will provide essential social services to patients in their homes, helping them navigate emotional, financial, and logistical challenges while ensuring compliance with Medicare home care standards.
Key Responsibilities:
Assess patients psychosocial needs and develop individualized care plans. Provide counseling and emotional support to patients and families coping with illness or life transitions. Assist with discharge planning and coordination of home care services. Connect patients to community resources, financial assistance, and support programs. Advocate for patients’ needs within the healthcare system. Maintain accurate and timely documentation in compliance with Medicare guidelines. Travel between assigned locations to provide in-home social work services. Masters Degree in Social Work (MSW) from an accredited program. Active state licensure (if applicable). Experience in home health, hospice, or medical social work (preferred.) Strong assessment, problem-solving, and case management skills. Reliable transportation for travel between patient homes.
