The Case Manager provides assessment, planning, linkage, coordination, monitoring, evaluation and advocacy to the child, family, foster family, and primary caregivers. The Case Manager works as a team member with the therapist, foster parents, primary caregivers, guardian’s ad litem, service providers involved with the family, and other appropriate professionals. The Case Manager will develop, strengthen, and maintain a direct working relationship with mental health professionals, community agencies and the child’s own support system. Essential Duties and Responsibilities Conducts home visits with children in accordance with federal and state statutory guidelines and OHU and CBC policy and procedure Maintains bi-weekly face-to-face contact with biological parents and assists them in reuniting with their children by providing appropriate referrals, support, and resources. Participates in Family Team Meetings as set for by the ECC Coordinator. Create case plans within 30 days of case transfer. After the initial case plan is created, the Multi-Disciplinary Team will collaborate to add items to the case plan that will address additional needs of the family as issues occur. Arranges visitation schedules structured upon the completed visitation plan with input from the appropriate parties, supervise visitations, and documents observations as appropriate Accompanies children to appointments, as necessary, to ensure their safety and well-being needs are met, and to report progress or lack thereof toward professional goals Ensures all medical, dental, and immunization records are recorded and uploaded into the child’s file in both FSFN and ASK Ensures all court orders, 5339s, and other related documents are completed within 48 hours of the decision of the child’s medical professional to provide psychotropic medication to the child Updates the child’s medications in FSFN on a monthly basis, at a minimum Assists the legal and biological parents, primary caregivers, and foster families in implementing tasks and activities according to the case plan Attends and participates in professional development trainings for professional growth as developed by the CBC, to include Pre-Service Training, as required by the state of Florida and in accordance with agency policy and procedures. Completes and attends all mandatory trainings. Achieves Florida Certification Board Child Welfare Case Manager certification within one year of passing the Pre-Service post-test. Abides by the Seven Professional Practices of the Child Welfare Model Attends and participates in staffing reviews, administrative reviews, judicial reviews, permanency staffings, school staffings, CPT staffings, meetings, etc. as required. Attend all family team meetings which will be held monthly prior to the court hearing for the family. Completes all non-clinical intake paperwork for case management services according to OHU and CBC policy and procedure. Develops and maintains appropriate documentation and concurrent planning in accordance with OHU and CBC policy and procedures Identifies strengths, limitations, risk factors, and needs, to refer and coordinate the provision of concrete services to the child, primary caregiver, and biological families of children Intervenes and assists OHU and CBC staff in crisis situations. Be trauma informed, have a strengths-based mindset, and encourage co-parenting Provides on-call support after normal business hours as directed by the supervisor, ADOP, or DOP Maintains client confidentiality according to established policies, federal, and state statutes, as evidenced by obtaining needed releases of information and maintaining updated releases of information Maintains established caseload productivity requirements and standards for direct client contact Maintains face-to-face and telephone contact with each of the assigned eligible children and families in the foster home, primary residence of the child and family, school, day care, preschool or any appropriate setting Notifies the biological parents of any changes in status, placement, medical & clinical treatment, etc., relative to the child and of all court hearings Provides a written, thorough assessment of each family and develops, with the team, a case plan in a timely manner of receiving a new case. Provides case plan updates and family support plans to all appropriate parties. Monitors and reviews case plans regularly with team members and properly files changes in the case plan in advance with the court, DCF, and the Lead Agency. Provides complete documentation for services rendered within established time frames, including daily log, progress notes, assessments and case plans, and other required documentation Documents all activities, to include client contact, service provider contacts, court hearings, telephone calls, home visits, parent and child visits, and any other activity related to the case within 48 hours in the Florida Safe Families Network computer system Develops effective and least-restrictive safety plans to ascertain the safety of children in both in-home and out-of-home settings. Maintains contact with all safety service providers and updates the safety plan when needed, and at a minimum once every 180 days Provides on-going support to the legal and biological parents, primary caregivers, and foster families to assist the family in providing a safe and secure home environment for the child Provides recommendations to reunite the family, terminate parental rights, remove children from families, and close cases when appropriate according to OHU and CBC policy and procedures. Work in close collaboration with the ECC Coordinator. Refers the child, as appropriate, for medical and dental care, including developmental, psychiatric, medical evaluations, treatment, etc. Tracks all important dates and attends court with prepared documentation. Testifies at court hearings when needed. Assists with notification of all appropriate parties to attend court and to attend all appropriate staffings. The Case Manager should be prepared to discuss the following at each monthly hearing, including but not limited to: visitation, sibling contact, services to parents, reunification, case plan amendments, and goal changes (must be raised explicitly in the notice of hearing sent to the parties unless notice is waived by the parties). Qualifications Education:
