PM Position Summary:

We are seeking a dedicated and experienced Travel Master Social Worker (MSW) to join our dynamic case management team. This position requires acute care hospital experience, with a strong emphasis on discharge planning and interdisciplinary team collaboration. The role involves managing complex cases, facilitating safe and efficient patient discharges, and supporting overall care coordination in a fast-paced environment.

Social Work Patient Ratios:

Complex cases (paired with RN): 8-9 patients Discharge Planning only: 25-30 patients

Team Environment:

Strong focus on collaboration with the multidisciplinary team (MDR/IDT)

Key Responsibilities:

Provide psychosocial assessments and discharge planning for patients Collaborate with medical staff, nursing, and other disciplines to facilitate effective care planning Coordinate post-acute care needs including home health, rehabilitation, and skilled nursing placements Use documentation systems to record patient interactions and discharge plans accurately Participate in interdisciplinary team rounds and patient care meetings Ensure compliance with regulatory standards and hospital policies Assist with crisis intervention and support services for patients and families

Qualifications:
Required:

Master’s Degree in Social Work (MSW) Previous experience in acute care hospital case management and discharge planning Knowledge of hospital discharge protocols and interdisciplinary team collaboration

Preferred:

Licensed Clinical Social Worker (LCSW) in the state of Florida Gross Pay (weekly): $1858.0 Taxable Pay (weekly): 612.0 Non-taxable Pay (weekly): 1246.0 #INDPRO
