This role supports a dynamic care coordination team and directly impacts patient outcomes by facilitating safe and timely discharges and continuity of care planning.
Unit Details Number of Beds:
344 (excluding Behavioral Health and Acute Rehab) For discharge planning only, caseload ranges from 25-30 patients
Documentation Systems:
MIDAS, Meditech, Nate, Navihealth, Care Team Assignment Call Requirements:
Must be available to work every third weekend
Floating Policy:
All social workers float except those in trauma
Patient Experience Focus:
Strong skills in hospital discharge planning and interdisciplinary team (IDT) collaboration Required Qualifications Active MSW (Master of Social Work) or LCSW (Licensed Clinical Social Worker) Acute care hospital case management and discharge planning experience Proficiency in MIDAS, Meditech, Navihealth, PASRR Ability to manage complex discharge coordination and collaborate with care teams
Preferred:
Familiarity with Medicare/Medicaid discharge regulations and transitional care planning Effective communication and problem-solving with families and multidisciplinary staff Adaptability to float across units as needed (excluding trauma) Typical Responsibilities & Duties Facilitate and document discharge planning for patients across diverse units Conduct patient and family consultations and coordinate post-acute referrals Participate in interdisciplinary rounds (MDR/IDT) Ensure compliance with payer requirements and regulatory standards Ideal Candidate Profile Organized and efficient in high-volume environments Compassionate and professional communicator Collaborative team player who adapts to workflow demands Why Fort Pierce?
