Family Preservation Case Manager – Child Welfare 3.5 3.5 out of 5 stars 901 Industrial Drive, Wildwood, FL 34785 The Family Preservation Case Manager is responsible for the provision of case management and support services for families in Circuit 5 and works in a team approach with the Family Preservation Specialist. This includes the completion of referrals to assist and stabilize the family, which includes, but are not limited to, mental health services, psychiatric services, housing, or other community referrals. Coordination with other agencies and community services is also required. Services are provided in the home to meet the unique needs of the families served.
ESSENTIAL FUNCTIONS FOR FAMILY PRESERVATION CASE MANAGER
Provides direct services to children and their families, which may include: Reviews comprehensive assessments with the family Reviews treatment plans and contracts for services with the family Establishes goal setting with families Manages and ameliorate crises with families Participates in discharge planning, which may include plans for follow-up treatment and connection to community services and programs Provides case management services Works with community groups in collaborative ventures Completes referrals for the family as needed Links the family to services as needed Completes documentation and reports in the appropriate reporting system accurately and timely Develops and maintains collaborative relationships with other community agencies and organizations to provide comprehensive and effective treatment services for clients (i.e., schools, courts, hospitals, etc.) Works as a team member with the Family Preservation Specialist and other staff to respond to the needs of the family Other duties and responsibilities as assigned. Attends all trainings and/or meetings hosted by Kids Central Other duties and tasks as assigned, as the above is not all-encompassing. This list of functions is not intended to be exhaustive. QUALIFICATIONS
Education/Experience Bachelor’s Degree in the human services field or BSW required and three years’ experience in a comparable position in a human services agency preferred but not required. Licensure/Certifications Current Background Clearance Screening Letter Valid Florida driver’s license Proof of vehicle insurance
