Videos Duties Social Work :

The CBOC social worker receives and completes consults from interdisciplinary teams and other social workers on complex difficult cases using advance practice skills and expertise. Incumbent is able to provide individual, group, and family psychotherapy and advanced level case management interventions used in the trement of veterans with polytrauma injuries, spinal cord injuries, traumic brain injuries, visual impairment, post­traumic stress disorder, addictions, and other serious disorders. Some of the duties include, but are not limited to; Provides consultion to referral sources and is responsible for development and maintenance of professional standards of practice and educing others regarding same. Provides educion on advance health care planning and facilites psycho-educional and support groups on a variety of topics for veteran and their families. Interviews and conducts psychosocial assessments to establish veteran’s trement needs, presenting problems, and DSM V diagnoses. Ability to expand clinical knowledge in the profession, provide consultion and guidance to colleagues, role model effective social work practice skills, teach or provide oriention to less experienced social workers, develop innovions in practice interventions, and provide clinical supervision for social work licensure or certificion. coordines screening of veterans for specific programs such as PTSD, Substance Abuse, MHRRTP, CNH, VIST, HHA, TCM program and provides ongoing trement and documention per VA Guidelines and facility policy. The selected Social Worker (along with the veteran) will develop the trement plan th includes measurable time limited trement goals. Collaborion with interdisciplinary teams such as TCM case manager, substance abuse staff, Psychiry (inpient/outpient), Homeless Program, Physicians and Vet Centers. provides independent consultion and makes recommendions to interdisciplinary team on course of trement. Acts as an advoce when approprie and feasible while encouraging the veteran and family to advoce on their own behalf thus fostering a sense of independence and empowerment. Makes rapid assessments and develops crisis management plans for admission to mental health inpient programs. Ensures th the interdisciplinary team is kept informed of any concerns with the pient and/or caregiver. Other duties as assigned.

Work Schedule :

The primary tour of duty M-F 8am to 4:30pm. However, the selectee may also be required to work various shifts on a permanent or temporary basis, based on the needs of the service and pient care.
