Jul 3, 2025 This position serves as the primary discharge planner for individuals who have been hospitalized for an extended period. This role will also require the ability to link individuals to appreciate services and resources in the community, complete assessments and referrals, and work collaboratively from a multi-disciplinary recovery perspective.
