Job Description
Social Worker Sawgrass Healthcare Inc 450 Northside-Cherokee Boulevard, Canton, GA 30115 Works with neonate, adolescent, adult, and geriatric patients, families, medical staff and community agencies to determine the appropriate setting of care and appropriateness of hospitalization, as well as to assess post-hospital care needs. Provides the necessary interventions in order to assure effective and efficient clinical resource management. Provides intervention regarding adjustment to hospitalization, crisis intervention and psychosocial assessment.
PRIMARY DUTIES AND RESPONSIBILITIES
: 1. Screens cases preadmission, admission, concurrently and post-discharge according to departmental polices and procedures. 2. Uses professional judgement as to appropriate interventions to impact clinical resource management. 3. Consults staff to identify and prioritize discharge planning problems in order to obtain accurate information about patient functioning and investigates post hospital care needs. 4. Furnishes outside reviewers with clinical information for reimbursement purposes to include retro reviews. 5. Interviews patient and/or family members to obtain information about the patient’s physical and mental functioning, family situation, support system and financial situation and counsels with patients, and/or family members to explore and decide upon post hospital care options. 6. Issues Notice of Non-coverage letters concerning transfer to private pay in accordance with hospital policy and Medicare and third party payor guidelines. Discusses financial responsibility issues with medical staff, patient and family members as appropriate. Contacts appropriate health care and community agencies and resources in order to facilitate and implement the discharge plan. 7. Documents assessments, progress and plan in patient’s chart, and keeps patient, family and staff apprised as to discharge planning progress and plan. Documents results of case management activities on worksheets. Makes referrals to Risk Management, Infection Control, Pharmacy and other areas as appropriate. 8. Receives referrals from Financial Counselors, physicians and nurses to assist families in matters relative to financial difficulties and directs to appropriate resource. 9. Reports possible abuse/neglect as per NSH Administrative Policies and Procedures. 10. Periodically updates resource file, including available referral agencies in the community, and maintains communications with outside referral agencies. In addition to the above mentioned duties, some Care Coordinator will be responsible for: 1. Responds to other in-house referrals, as well as outside calls, and provides appropriate resource information and referrals. 2. Implements studies to review appropriateness of social work services using pre-established criteria. Identifies, researches and develops, if necessary, support, informational, and educational materials as aids for working with assigned population. 3. Providing in-service for hospital staff on case/clinical resource management. 4. Participates in appealing denials attributed to Care Coordinator. In addition to the above duties, all Care Coordinators will be responsible for: 1. Practices proper safety techniques in accordance with hospital and departmental policies and procedures. Immediately reports any mechanical or electrical equipment malfunctions, unsafe conditions, or employee/patient/visitor injury/accident to Manager.

$50.00 – $55.00 per hour Expected hours: 40 per week

8 hour shift Monday to Friday Weekends as needed
