Responsibilities:
Assess patient needs (neonates to geriatrics) and coordinate discharge plans Provide crisis intervention, psychosocial evaluations, and support for patients/families Communicate with care teams, payors, and community agencies Maintain detailed documentation and support case management reviews Address financial and social barriers to care
Min Requirements/Qualifications:
