Responsibilities Conduct patient assessments to determine needs and develop individualized care plans Provide counseling and support to patients and their families Collaborate with healthcare professionals to ensure comprehensive care for patients Maintain accurate and detailed patient records Utilize medical terminology in documentation and communication Collect and analyze data to monitor patient progress Assist in crisis intervention when needed Work in hospital settings to provide social work services
Requirements:
